I need help to design this an evaluation form please.

Design an evaluation form that you and the other panel members will use during the selection interviews. The evaluation form should contain all of the information panel members will need during the interview to ask questions, evaluate responses, and take notes. You are essentially creating a form that will contain all of the work you completed in Part 1 of this project in a format that is user-friendly for the panel members and includes pertinent info (name, date, interviewer, etc). Lastly, include a section on the form for evaluating other attributes the panel members should be considering when assessing each candidate that may not be addressed by the interview questions, ie/ professional dress, level of interest, positive attitude, etc. This project requires research and references. Please do not submit work without doing the research required to make sure you meet the Human Rights Laws and legal requirements
