I need help to make a corrective action regarding this incident as one of my worker got injured during work incident.

the root cause is under investigation i dont know what happender and not in the seen

heres my Incident Report,

Description of Incident:

On Feb09/2023], at 0630pm our worker marilou, was vacuuming the hallway outside a small meeting room in [location] when suddenly, the double sliding door fell and hit her head. She did not have any visible cuts or bruises, but she decided to go to the doctor as a precaution. She did not receive any first aid, but she removed any small pieces of glass from her clothes and then sat for a few minutes.

According to the worker, she did not touch the door before it fell and broke.

Actions Taken:

The worker was advised to seek medical attention as a precaution.

Recommendations:

It is recommended that an investigation be conducted to determine the cause of the incident, and that appropriate safety measures be taken to prevent a similar incident from occurring in the future
