I would like help with building a php

html site that has three pages and connects to a SQL database.

The first page should be a welcome page that has a button that links to the next page.

The second page should have a form where data can be entered and added to the database.The submit button should take the user to the confirmation page.

The third page is the confirmation page that should display the results of queries made on the data. If possible it would be nice to have a home button that goes back to the first page.
