Though the driver managed to avoid a collision, the incident resulted in minor injuries due to the sudden stop. The preliminary investigation highlighted that the truck had missed its last two scheduled maintenance checks. Further, it was found that the driver had worked beyond the recommended hours, contributing to fatigue and delayed reaction time.
