SURVEY QUESTIONNAIRE MAKING

YOUR TEAM HAS BEEN MADE BY THE INSTRUCTOR. THE TEAM MEMBERS CAN BE FOUND ON A SEPARATE PDF ATTACHED WITH THIS DOCUMENT.

The Situation:

The management team at XYZ School has contracted you to construct a survey to gauge your target markets attitudes towards a particular burning issue within their high school.

Your Task:

You will design a survey with the objective of researching a hot issue within our school that you feel needs attention. Some possible topics could include:

XYZ Schools Cafeteria

XYZ Transportation System

XYZ Schools school uniform

XYZ Schools students working part-time and its effect on their life Relationship between studying and grades

Bullying

You can even choose your own topic

For each issue, you must come up with a focus or objective for the survey (For example. what is it you really want to find out?). You need to write down the focus/objective on the top sheet of your answer. Once youve created and started this focus, from the next page you need to start writing the survey questions.

The survey should be trying to accomplish this task by asking questions that are effective in determining the following:

1. Do people have an interest in the issue?

2. What are peoples current attitudes towards this issue?

3. Why are they happy/unhappy with the current situation?

4. What types of change would students like to see occur?

5. How likely is it that the change will occur?

Your questionnaire should include a minimum of 20 questions (maximum 25 questions) that effectively question your intended target about your objectives.

SURVEY QUESTION TYPES TO BE INCLUDED IN THE SURVEY:

1. OPEN-ENDED

2. CLOSE ENDED

3. LIKERT SCALE / SEMANTIC SCALE

4. MULTIPLE CHOICE

5. ORDINAL

6. CATEGORICAL

7. NUMERICAL

Check your survey for required components:

Do you have a minimum of 20 questions?

Do you have a minimum of 6 different styles of questions?

How many open-ended and closed-ended questions do you have? Do you have a good mixture/variety of both types?

Do the questions follow the sample formats exactly? I.e. boxes to check, lines to fill in, bubbles to complete?

Are all of the questions necessary and relevant to prove your topic? I.e. if age or gender is not relevant then do not ask for this information.

Are there enough possible answers to select from (in the closed-ended questions) for the user to accurately respond?

Are the questions written in an unbiased manner? Try to follow the Do and Donts of the questionnaire making.
