Most accidents are the result of a chain of errors that accumulate and compound to lead up to the final error that actually results in the accident. If that chain of errors is interrupted, then the accident is averted.

Traditionally, pilot training has been maneuver-based where the training and evaluations are conducted by individual maneuver-to-maneuver.

The FAA has developed the Advanced Qualification Program (AQP). This is a voluntary program that is available to Part 121 and Part 135 operators.
