1. Name of the fictional company (address, phone number, etc.) 2. Background company information This includes informing the facilitator of the nature of the company What does it do? (manufacturing, service, etc.) (This is a short narrative) How it does it? (General overview of how it makes its products or provides services.) (This is narrative) Identify departments, and number of employees, (additional pertinent company information needed to understand the company) Surrounding companies and environment (Explain what companies are in the surrounding areas. Short narrative explaining them.) 3. The purpose of this EAP proposal is for the facilitator to be able to understand fully the company while you are drafting your final EAP.
