It contains at least four of the following types: Text, Email, Menu, Radio Buttons, Checkboxes, Submit and Reset buttons.

The content of the form is meaningful and related to your website topic.

The page containing the form is integrated in the website design (i.e., has the websites style and navigation).

Submission of the form causes the form data to be emailed to you.

E-mail and at least three additional data types should be required and checked by the form validator.

After the successful submission for information the user needs to be redirected to a Success page.

The Success page is integrated in the website design (i.e., has the websites style and navigation).
