Assignment Instructions:

Select a System/Process:

• Choose a system, product, or process relevant to your field or current project.
• Define the scope clearly, focusing on a subsystem or specific process stage if necessary.

FMEA Development:

• Identify Failure Modes: Break down the chosen system/process into its main components or steps. List potential failure modes for each part.
• Analyze Causes and Effects: For each failure mode, identify potential causes and assess how each failure might impact the overall system or process. Be specific about how failures could affect safety, performance, cost, or customer satisfaction.
• Assign Severity, Occurrence, and Detection Ratings:
  • Use a 1-10 scale for severity, occurrence, and detection, where 10 is the highest risk or difficulty to detect.
  • Calculate the Risk Priority Number (RPN) for each failure mode (Severity Occurrence Detection).
  • Prioritize the failure modes based on their RPNs.

Risk Mitigation Strategies:

• For failure modes with high RPNs, propose specific strategies to mitigate risks. These can include design changes, quality control measures, process improvements, or maintenance strategies.
• Justify each recommendation with supporting reasoning and how it would effectively reduce the identified risk.

• FMEA Worksheet: Submit a detailed FMEA worksheet listing all failure modes, their ratings, RPNs, and proposed actions.
