This week's discussion is based upon the video Case Study No.1: 'Crossed Wires'

As you watch the video, consider what you've learned in this module related to ETBA and risk assessment; identify the hazards relating to the procedure being performed. In your initial post use the risk assessment format in Chapter 8 of the textbook to explain the cause and effect of the identified hazards.

In addition, provide a recommendation for mitigating one hazard identified by classmates based on the system safety precedence provided in Chapter 2 of the textbook. Identify the ETBA hazards by connecting them with risk. Explain why the mitigation method was chosen.
