Pick a test and prepare a package that you would provide to a patient with information on their test. Alternatively, you can choose to orderblood tests and urinalysis on one requisition.

The package should include

• A lab requisition
• Time date and Location of my test(use somewhere local to you)
• Contact information of test site in case of changes
• What I need to do to prepare for my test
• What to expect before, during and after my test
• How long before I receive my results
• How will I receive my results.
• Include what symptoms the patient is experiencing and what exactly the test is looking to clarify. Are companion tests necessary, blood work?
• A REFERENCE.
