Write down five (5) falls-prevention Intervention strategies for the client depending upon your falls risk assessment. You should follow the organisational policy framework to do this plan. You should do these activities in a collaborative, positive and respectful manner. Expected outcome section should include when the intervention ceased and/or monitoring of falls prevention strategies and interventions.

Write down two (2) assessment findings for the client depending upon your monitoring of falls risk assessment You should include feedback from the client and carers on intervention strategies used. The results of the Intervention (Successful or unsuccessful) for each of the assessment findings and interventions used. You should refer the client/resident to the other health care team members or agencies if required You should follow the organisational policy framework to do this plan. You should do these activities in a collaborative, positive and respectful manner.
