Today she presents to the Pediatric Clinic for a scheduled follow-up of her referral; she requires a history and medication review, as her file is incomplete. The MD noticed she has not been seen by OT or PT services within the last year. The MD has ordered both PT and OT Initial Eval and Tx. Family information to date includes that she is has no interest in reading, although appears bright and has an immense memory for anything she hears.
