Question:
Poor Mr.Wang He has been struggling with his injuries. The skin on his hands degenerated and required skin grafts. The skin graft on his dominant hand got a staph infection. After 8 months of multiple surgeries and IV antibiotics, that hand was amputated. Additionally, the antibiotic that he was given (vancomycin) caused permanent kidney damages and he now has stage IV kidney failure.
His retinas were damaged by the fire and has damaged his visual acuity. His "good" eye is measured at 20/200 with the use of glasses.
You have helped him so greatly with his other claims. He would like YOU, the paralegal, to represent him in his social security claim. He has already applied and been denied twice (once at the initial application level and once at the reconsideration level.)
Fill out the Request for hearing, (only that document- not the other medical history etc)AND any representative information that you may need.
ALSO prepare a prehearing statement/brief arguing his claim.
