Secure enrollment form at the entrance area b. Accomplish enrollment form with other forms attached and have it been approved by the department chairperson or person- in-charge c. Submit accomplished enrollment form to the registrar's office for encoding d. Proceed to the Registrar's Office for the issuance of class cards Via Mobile SMS a. Initial SMS, send a message (from 8:00 am to 5:00 pm only) to a registrar's mobile numbers provided. Include the following information: . For Returning Students (Complete name, Permanent Address, Birthdate, Birthplaces, Semesters last attended, Course Major and Year Level ii. For New Students (Birthdate, Birthplace, Permanent Address, name of parents, contact number of parents, senior high school Graduate Form, LRN if Applicable) b. Verification of Sender's identity c. Evaluation and Encoding. Upon verification, the person in charge of the enrollment form will encode subjects and other personal information. d. Approval by Department Chairperson e. Notification, Status of enrollment will be relayed to the enrollee Recommendation
