Write a paper of at least 4 pages of WRITTEN text (cover page, heading and citations page don't count) explaining the data and quality tools you would use to analyze the quality improvement problem you identified in Part 1.which is (CDCs definition focuses on activities that improve population health, ensure healthcares affordability, and deliver the best patient experience. These three dimensions mirror The Institute for Healthcare Improvement (IHI) Triple Aim; the framework all quality improvement in healthcare ties back to:Improve the health of populations.Reduce the per capita cost of healthcare.Improve the patient experience.The Triple Aim is a framework for optimizing health system performance. And the primarygoal of quality improvement is to improve outcomes. CDC also describes quality improvement as one component of the performance management system, which has three defining characteristics: It uses data for decisions to improve policies, programs, and outcomes. It manages change. And it creates a learning organization). Your paper must contain the following 4 elements. Use these 4 elements as the outline for your paper. Be sure to adhere to the writing standards (cover page or section, citations page, page #'s etc.) as described in the syllabus.

1. BRIEFLY (1 paragraph) restate the quality problem you selected in Part 1. Do NOT copy / paste from Part 1. Instead, revise and summarize your problem statement using information and terms you have learned to date regarding quality such as the cost of poor quality, customer perspective, sources of waste, risks due to poor quality, etc.

2. Collect data (numbers, dates, counts, etc.) that quantifies the quality improvement problem and identifies key failure points and opportunities for improvement. If you cannot collect actual data, think about what type of data you WOULD collect. Explain the data that you are going to collect/have collected or would collect and why you chose this data. How often would you collect the data? How many people or events will be included? How will you record the data?

3. Explain how you would use your data to employ one of the Seven Continuous Improvement Tools described on Page 160 168 of Chapter 4. The Seven Continuous Improvement tools are: flowcharts, check sheets, histograms, Pareto diagrams, cause-and-effect diagrams, scatter diagrams and control charts. Select one of the Seven Continuous Improvement tools and explain why you selected it. Explain what useful information the tool provides and how it informs you further about the root cause of the quality problem you are analyzing.

4. Apply the continuous improvement tool to your quality problem. Create and include an example of your selected continuous improvement tool in your paper using the data you described in step 2.

NOTE: Do *NOT* propose solutions to your quality problem in this paper. Part 2 is a deep dive into collecting and analyzing data that helps you understand the root causes of your selected quality problem and will inform you of possible solutions. The solutions will be the focus of Part 3 so do NOT write about recommendations in this paper! Save that for Part 3.
