Question: A bank manager is concerned about the performance of his employees. Some of them perform very satisfactorily and some others are poor performers. By performance, he has in mind such things as the quality of their work, the quantity of work done by them, their knowledgeability about the bank's policies and procedures and the accuracy of the information they provide for the bank's customers, etc. He wants to know two things. One, how the employees rate themselves and how their supervisors rate them, and two, what demographic or personal data--such as age, for instance--would influence performance.

Design a questionnaire for this purpose.
