Kostya

Kostya is an 18-month-old boy who has been adopted from an orphanage in Kazakhstan. Kostya's adoptive moms, who identify as lesbian, are pediatrician Renee and attorney Jen. Both are high-achieving and affluent, and Renee falls on the high end of the autism spectrum. The couple struggled with IVF treatments using donor sperm in their late 20s and early 30s. They have one other child, 4-year-old Ainara, who was also adopted. Due to the years of fertility challenges and associated emotions, Renee and Jen have a strained relationship. In addition, they hold different views about how to navigate the challenging behaviors displayed by both of their children. When Kostya was 16 months old, Renee and Jen traveled to Kazakhstan to meet Kostya, interact with him under the supervision of the orphanage at which he was living, and to finalize the adoption. A short 4 weeks later, Kostya was adopted and returned with Renee and Jen to the United States. Now that they are home, they have a nanny who assists with the care of both children due to the parents' demanding jobs. Kostya will soothe with the nanny when he is dysregulated; however, he will not soothe and calm down with Renee. Renee states, "It is like nothing I can do is right. He just cries more. I don't think he really likes me, and I don't know what to do. It often ends with both of us crying." Jen works long hours and states, "It is easier to deal with the stress of work than to be home and deal with stress I don't know how to fix." Both Renee and Jen report concern about Kostya being a quiet child who watches TV for long periods of time. Additionally, Kostya has difficulty interacting with caregivers and other children his age, is picky about the food he eats, which is very little, and has difficulty staying asleep at night.
