Explain how this model is advantageous to patient outcomes" Araceli's post An example of a healthcare delivery model that utilizes interdisciplinary care delivery teams is the Patient-Centered Medical Homes (PCMHs). The model emphasizes the coordination of care from different health care professionals who work together to manage the overall health needs of a patient. In this model, the patient has a central primary care practice which acts as the center point of the health care. With this model the patient has a ongoing relationship with a physician who provides continuous and comprehensive care (O'Dell, 2016). That same physician leads a team of individuals who all share responsibility in caring for the patient (O'Dell, 2016). The primary physician is also the one responsible for interchanging other qualified professionals are needed for the patient such as social workers, dietitians, case managers, etc (O'Dell, 2016). This model is advantageous because it promotes a collaborative approach to healthcare. This allows better management of chronic diseases and comorbidities, provides better access to care, and enhances communication with professionals (National Committee for Quality Assurance, n.d.). Because the model puts patients at the forefront of their care and promotes positive relationships between the patient and healthcare team, these patients have better experiences and report increased satisfaction with their care (National Committee for Quality Assurance, n.d.). What is a Patient-Centered Medical Home Tenzin's post The growing demands of aging populations and chronic illnesses are transforming acute care delivery. Hospitals and governments are adopting innovative, patient-centric approaches, including cost-effective care alternatives, digital services, and consumer engagement. New technologies and value-based care models empower patients to make informed decisions, while healthcare systems emphasize holistic, collaborative approaches to meet complex care needs with greater convenience, transparency, and personalization (Edelmann, 2023). There are many innovative health care delivery models like Value-Based Healthcare, Collaborative Care, Advanced Primary Care, Virtual and remote care and the shift towards home care, Digitalization and Digitization: enablers of the new model (Edelmann, 2023). One such model called the Patient-Centered Medical Homes (PCMHs) is an interdisciplinary care delivery model where clinicians and staff work at the top of their skill sets, supported by strong relationships with patients, families, and caregivers. This model emphasizes accessibility, comprehensive care, high-quality coordination, cost-effectiveness, and patient-centered focus. PCMHs provide easy access to care, including after-hours support, and influence multidisciplinary teams to deliver holistic treatment. They emphasize quality over volume, incorporating data-driven practices to enhance patient outcomes while reducing costs, as evidenced by lower emergency visits and hospitalizations. Centered on individual patient preferences and values, PCMHs represent a collaborative, innovative approach to addressing healthcare challenges. The model ensures seamless care transitions, improved accessibility, and patient empowerment through holistic, preventive, and coordinated care. It enhances patient satisfaction, reduces hospitalization, lowers costs, and supports better population health while prioritizing individual well-being (Advanced Medical Reviews, 2017). The innovative healthcare delivery models that will drive patient-centered care. https://healthcaretransformers.com/value-based-healthcare/innovative-healthcare-delivery-models-driving-patient-centered-care Michael's post We are discussing new and innovative approaches in health care. I strongly believe Al technology with clear and transparent guidelines, will significantly impact the way we provide care across the life span. Using pattern recognition, Al can positively affect the healthcare environment and the interdisciplinary team to help make more informed decisions which will help patients continue to take an active role in their health by automating repetitive task which allow providers to focus on higher cognitive task that would limit built in bias for patient care (Hazarika, 2020). A new area of specialty is emerging in this area. The system wide benefits and pitfalls need to be considered as a part of innovative systems. Consider an Al assisting with home care. It can ask the patient to confirm that they are taking their medications, reminding to eat and drink, monitoring safety and reporting potential falls, providing soothing favorite music, and record pertinent data that can be electronically delivered to health care professionals or law enforcement or fire departments or a plethora of SDOH organizations that can assist the at home patient with various needs. Now consider the development of the android that can become a caregiver with preparation of meals, company to assist with social interaction that helps maintain cognitive function, and so very much more. This becomes an area of great concern for ethics, workload, labor issues, quality of performance, satisfaction, professional liabilities, provider competencies with a potential decline in application of higher cognitive decision making for the patient, regulatory compliance, and provider patient relationships (Hazarika, 2020). So many applications with so much to consider. There will be questions that we can not begin to consider. However, we should start this conversation because in the next 25 years, this will be employed and the impacts should be discussed. Reference Hazarika, I. (2020, July 1).
