Section 2. 1. Read the Guidelines at the beginning of the Evaluation and Management (E&M) section of the Current Procedural Terminology (CPT) book. 2. Read Kinn's book, Chapter 13. 3. Download then complete the Evaluation and Management worksheets. 4. Place your answers in the spaces below. Each problem has only ONE answer. When attempting to code services from the E&M section 11 - Determine where the service was provided. The locations of the provided services for this activity are in bold below. Practice locating them in the subsection in the book. Not all locations are represented in the worksheet activities. 2nd - Determine if the patient is new or established. In some cases, this information need not be distinguished to accurately code. Double-check that you are in the correct subcategory when coding with the CPT. 3rd - Determine the history and examination level. Very often, they are the same. Manually determining the levels of history and examinations can be very difficult and time-consuming. Electronic medical record systems have the ability to automatically calculate the level based on the encounter documentation. For the purposes of this exercise, the levels are spelled out for you within the problem. 4th - Determine the complexity of the medical decision making (MDM). Like history and examination levels, the complexity of MDM is also difficult and time consuming when done manually. The worksheet problems will also have these complexities spelled out for you.
