Task 3 (AC3.1, 3.2) Care plan for a vulnerable individual Read the task 3 case studies. For one of the individuals, create a plan that can be shared with the team who will be involved in their care to explain communication strategies that should be used with the individual whilst they are in the health and social care setting. The plan should include: Strategies for day-to-day communication with the individual about their personal care needs (e.g. meals, hygiene, how they will spend their time between treatments] Strategies for ensuring that the individual understands their treatments and the next steps Strategies for communicating with the individual if they become distressed The plan should include analysis of why you think the chosen strategies will improve communication with the individual. Include your word count at the end of the work, Your plan does not need to be in continuous prose you can use tables, bullet points or a mind map to organise your strategies Sources should be referenced in-text and in a reference list. The references should follow Harvard style conventions. The reference list is excluded from the word limit. Case Study 3: Danesh Danesh has retinitis pigmentosa, a progressive condition that leads to sight loss. He began to experience sight loss as a teenager and now, in his thirties, has been registered as blind. He has found it difficult to find a career that he finds fulfilling and has experienced periods of severe depression, treated with antidepressants and talking therapies. Recently, a mole on his face has grown and his GP has referred him to a dermatologist to check for melanoma. The dermatologist will conduct a biopsy. If the biopsy is positive for melanoma then Danesh will require further tests and surgery
