- Team Assignment: QI/PI Situational Analysis Identify a performance or system-related problem you might encounter in your hypothetical organization. The problem should be one that can be addressed through improving staff performance and/or redesigning processes or systems. For example, medical errors, long wait times, language barriers, unclean equipment. Focus on one specific problem. 5 pages ddress each of these items: 1. Describe the problem in one or two paragraphs (This is the narrative background for how and why you chose the quality/performance improvement issue. What is the current situation?) State the specific division, department, or program description. 2. Problem Statement (one sentence) What is the performance problem the organization or department wants to improve? 3. Plan for improvement (specific action steps; who, what, when, where, how?) 4. Include at least one diagram, e.g., fishbone, flowchart, Gantt chart, to describe the problem and/or solution 5. Plan to measure results of improvement plan
