Termination appears to be a straightforward process as work with a client comes to completion. The text outlines a Discharge Summary form to be used in this procedure. When all the work is complete, and the client is happy and has made great progress, this goes smoothly. However .. too often insurance, finances, lack of motivation or progress can also be reasons that a client discharges from your care.
