Using a single SQL query, I want you to provide me with the following set of summary information from the Instructors table in the MurachCollege database. 

The first column should provide the last name of the instructor (column titled Last Name).

The second column should provide their monthly salary calculated from their annual salary (column called Monthly Salary).  Assume that the instructors work and are paid equally all 12 months out of the year.

The third column should provide a confirmation that each person in the result is a department head (column called Dept. Head).  This is a constant value.  Use English (e.g. 'Yes') rather than the '1' designation in the DepartmentChairman attribute.

The query results should only consider instructors that are department heads.  Organize the result first by monthly salary (from largest to smallest) then by last name (from A to Z).
