Using the Blue Cross/Blue Shield EOB, why does Mark Nelson owe more money than just a copay?

1)	Patient only owes a copay and the claim should be appealed

2)	Patient has an 80/20 plan and has met the deductible for year

3)	This has been calculated incorrectly. Amount due by the patient should be calculated by the amount charged and not the amount allowed due to an 80/20 plan.

4)	Patient has an 80/20 plan but has not met the deductible for the year. Once the deductible is met, he will only owe a $30.00 copay
