Care Clinic, which is private health care facility, has two clinics in different locations. It deals with patients, doctors, nurses, and medical procedures, or services. The clinics have three rooms each for examination and treatment. One room has facilities for the doctor carry out minor operations, and this room also used in cases where patients have to receive drips, etc.
A patient may visit a doctor anytime, and can request for an appointment as well. When a patient requests an appointment, a visit is scheduled for a particular date and time with a specific doctor. The patient's history record is checked to see who the patient's usual doctor is and check that person's availability. Each of the doctors has their own appointment lists for each day. An appointment list simply lists all the scheduled appointment times and patient names, along with the codes of the services that are to be performed for each patient.
There is one appointment book for each doctor in each clinic. An appointment book lists times throughout the day in half hour blocks. Certain times are blocked out in the doctor's book, for example, lunch times, days off, vacations, conferences, and so on.The rest of the times are available for appointments. Every day, an appointment list is prepared for the next day for each of the doctors. A call list for the day is prepared after that. All patients on the call list are telephoned and reminded of their appointments. When a patient has been contacted, his or her name is crossed off the call list.
Whenever an appointment is completed, the doctor writes on the patient medical history sheet details of the procedures that were performed and any medication prescribed. The doctor brings the patient's file out to the receptionist. If the patient wants to pay for the services immediately, the receptionist calculates the total charges, makes a note of the payment on the sheet, and prepares a receipt for the patient. If the patient is covered by insurance, the insurance claim forms are filled.Every night, all the insurance claims are mailed.
The patients are insured through different insurance companies. A file is kept on each of those insurance companies, showing details about submitted claims and the payments received on claims. Patient's employer details are also recorded, including the full company name, address, and the group number of their insurance coverage plan.
There is one folder for each patient. The patient folders are kept in alphabetical order by the patient's surname. Within each folder there are three different kinds of files or records. The first is a sheet of paper on which the name and address, his or her employer and insurance company (if any), and work and home phone numbers are present. The second kind of record is the patient charge/payment record.Any time we treat any patient, an entry is made showing the date, service, charge, and new current balance. If the patient has insurance coverage, a check mark is made next to any charge for which a claim has been sent to the insurance company. When payment is received, an entry is made in the patient charge/payment record showing the source, date, amount, and new current balance. The third item kept in a patient folder is a patient medical history sheet(s), which has list of every procedure performed on the patient, the date the service was performed, and the doctor is recorded. The patient's chart, which is maintained by the doctor, and any additional notes also are in the patient's file.
CareClinic wishes to automate all these processes, except handling insurance payments,which will be done manually. The receptionist should be able to use system to record, maintain appointment list, and call list. The system should also handle payments, and allow the doctors to enter the details of procedure performed on the patients. The new system should also allow patient records to be kept in both clinics. This means that a patient can visit any of the two clinics and when doctors look at their history, they get the up to date data.
