He is currently residing at a skilled nursing facility. He has history of parkissons disease. disease. Assessment was done in the patient's room. He said his sleep has been erractic the past couple of weeks. He said he wakes up at night and cannot fall back asleep. He said last night, he was up by 3am and could't fall asleep again. He is married, has a supportive wife, 4children and 6grandchildren. Current meds are Carbido-levodopa 50-200mg (take 1 tab TID), Carbido-Levodopa 25-100mg )give 1.5 tab in the am for parkisson's disease
