Write an Initial PSYCH VISIT, chief complaint, , Psychiatric visit HPI, current Psych meds, exam, social history,, screening tools and results MSE, diagnosis and recommended Plan (medication and dosages), rationale for using the medications for 78 years old female referred to be seen for initial psychiatric visit because resident usually had in person therapy prior to admission and finds new telehealth video couselling therapy unhelpful. Her PHQ 9 AS OF 6/11 WAS 7/27. Assessment was done in the patient's room. She said people she has never seen before keep walking in saying they are her doctors. She said she prefers her counseling in person and not someone just calling her on the phone who does not know her history. She said someone from conseert health therapy just called her on the phone and she was not even informed, and to mkake it worse it was just a phone call and not a video for the therapy. She was anxious, pressured speech, want things in a specific order as noted during the assessment. She said she has her out of facility therapist for years and she trust her more because she knows what she is doing. Se currently has orders for Hydroxyzine 25mg PRN every 8hrs for anxiety started 6/11/25 and stop date is 6/25/25. She said Hydroxyzine makes her more irritable but records indicated it was administered to her on 6/11. Keep hydroxyzine orders till 6/25
