Write this report in a proper professional manner following the investigation steps in environmental health when investigating an incident for food poisoning case (paragraph format) Highlight all the steps discussed and modified them.To report on alleged food-poisoning noodles resultant on death of three Ngwendu children and to identify the possible source/cause of the death guided by Environmental Health legislations and by-laws pertaining to food safety and to provide recomendations.
BACKGROUND
A complaint on alleged food-poisoning noodles that said to be the cause of death of the three Ngwendu children was received by this Sub Directorate on the 08th November 2021 telephonically from Dr P.Nodwele.An investigation thereof was conducted on the same day by Miss K. Sibeja an Area Environmental Health Practitioners and a Principal Environmetal HealthPractitioner,Mr R.Blignaut.At the shop where the investigation started we met the area councillor Ms K.Ngxenge,Mr Duba from SANCO,and community members and the Media crew (not sure from which company as there was no communication with them).
FINDINGS
The children bought the noodles at the local spaza shop located at no 80 Tambo street in New brighton.
The shop where the noodles were bought does not have a valid business licence and certificate of acceptability therefore it is operating illegally.
The company manufacturing the noodles alleged to be bought by children is Grandisync company,located at Uitenhage within Nelson Mandela Bay Municipality,they are manufacturers and distributing the product nationally and in possession of certificate of acceptability,Ms Vawda (representative of the manufacturing company) indicated that they are not testing their products more often and when they do they send the samples to Dr Mauritz Wenzel lab in George.
Children that were affected were residing in Wells estate at 32 Zadunge street,However they visited a relative at No.11 Goniwe Crescent, New Brighton,where they ate the noodles but developed the symptoms on their arrival at home in Wells Estate.
Before the death they are said to be complaining of stomach cramps,weakenes and had foam coming of the younger childs mouth and died on their way to Motherwell health care center.
The community members shared that the grand mother of the children was hospitalised at Motherwell health care center on the 7 of November 2021and his brother Mr Xolile Ngwendu was contacted and he confirmed that it was due to shock and was out of the hospital the same day.
Environmental Health Practitioners visited the house at 11 Goniwe Crescent, New Brighton on the same day and no one was found.
On the 16 of November 2021, EHPs revisited the house at 11 Goniwe Crescent, New Brighton and found Mr Ngwendu and he mentioned that the children were accompanied by their 14 year old cousin sister to buy the noodles but she didnt stay left them alone.We didnt get hold of the cousin to find out if she also bought herself the product.
No Toxic chemical substances at the house were found/observed.
Food preparations (noodles) were done without any supervision by the 11 year old,Sinothando.
The shop owner bought seven boxes of Howe instant noodles at Discount wholesale in Korsten and each box had (10x5) of 75g instant noodles all sold to community members.
On the 17 of November 2021,Environmental health team conducted a survey on 15 households around Tambo area about the noodles,no one expirienced any food-poisoning related symptoms after consuming the Howe instant noodles.
On the 18 of November 2021 Mr Baadjie (Manager in charge) at NewBrighton Clinic was contacted and He confirmed that there were no patients around Tambo surroundings visited the clinic complaining food poisoning related symptoms
On the 18 of November 2021 the result sent for bacteriological analysis on the product from National Health Laboratory Services were received and revealed that there were no pathogens present on the noodles, however the mould was detected.

ACTION TAKEN:
On the 8th November 2021:
Howe instant noodles 75gx8 were taken from the shop for food analysis and 75gx4 was sent to NHLS (Port Elizabeth) for bacteriological analysis and another 75gx4 was sent to Cape Town for chemical analsysis on the 8th November 2021.
The shop was closed due to non-compliance to environmental health legislations and re-open because we didnt leave the closure notice.But currently the shop is closed.
