You observe a female co-worker "Sally" who has become friendly with a 15 year old male client "Joshua". They have added each other on Facebook and Sally has given Joshua her phone number. Joshuamentioned that he called the Sally on her day off "just for a chat". When you mention this to Sally she says "oh yeah no big deal". When Sally is on shift, she often gets caught up talking to Joshua and doesn't pay much attention to other clients. Once when an incident occurred between two other clients, which becomes aggressive, you look for Sally for back up, but she is outside talking to Joshua. The workplace policy is that of two staff on the floor at all times.
