Wickham Terrace Private Hospital (WTPH) is seeking a hospital information system (HIS). WTPH offers theatres and wards for use by various doctors. Each doctor has a speciality. WTPH is equipped to support a limited range of specialities including:

Orthopaedics

Vascular surgery

Obstetrics and gynaecology

General surgery

Plastic and reconstructive surgery

The HIS is to store the details of doctors, patients, and hospital admissions. Each doctor must have a speciality supported by the hospital. Each doctor has exactly one speciality. Prior to the patient arriving at the hospital, WTPH communicates with the prospective patient to collect patient details. These details are recorded on the Pre-admission form (see Appendix B). As any patient may be admitted to the hospital more than once, on their first admission, the hospital allocates the patient a unique patient identifier, the Unique Record Number (URN). Internally, each admission for the patient is given a sequence number (starting at 1). For any admission, the patient will be under the care of a single treating physician. The hospital has 5 wards, each consisting of 10 single rooms and 5 shared rooms. Shared rooms can accommodate 2 patients at a time. The hospital also has several operating theatres, a pathology lab, imaging facilities (x-ray, ultrasound, CT, MRI), and a pharmacy. The pathology lab conducts blood and fluid testing. The pharmacy supplies medications to patients during their hospital stay and will provide any medications prescribed by the patients treating doctor on discharge from the hospital. Treatments, procedures (such as surgery and medical imaging), provision of medications, etc are identified with a unique code, and each has a price. Each surgical procedure is related to a particular speciality and can be performed only by a doctor with that speciality. During a patients admission, each activity performed by a caregiver is recorded in the patients chart. A new chart is created for each admission by the same patient. On discharge, the hospital invoices the patient for the billable services provided as well as for accommodation if an overnight stay is required, i.e. length of stay is greater than one day.

Deliverables You are required to submit the following items for Task 1:

1. A list of elementary facts that fully and explicitly verbalise the fact types contained in the scenario description.

2. A conceptual schema model produced using the Entity Relationship Diagram (ERD) language, using the Barker notation, which fully captures all of the fact types and any necessary constraints.

3. A conceptual schema model produced using the Object Role Modelling (ORM) language, which fully captures all of the fact types and any necessary constraints.

4. A textual description of the ERD model, prepared for the business owner, as an aid to communicating and understanding the less obvious features of the model

5. A textual description of the ORM model, prepared for the business owner, as an aid to communicating and understanding the less obvious features of the model
