All plans in the state exchanges must offer the following benefits:

• Ambulatory patient services

• Emergency services

• Hospitalization

• Maternity and newborn care

• Mental health services and substance use disorder services, including behavioral health treatment

• Prescription drugs

• Rehabilitative and habilitative services and devices

(rehabilitative therapies improve, maintain, or prevent deterioration of functions that have been acquired [e.g., after an adult has surgery], while habilitative therapies are provided to achieve functions and skills never acquired [e.g., as with a developmentally disabled child])

• Laboratory services

• Prevention and wellness services and chronic disease management services

• Pediatric services, including vision and dental services
