Case 1 A Failure in Communication It is thought that a failure to communicate might have contributed to the space shuttle Columbia's crash. Several senior NASA engineers had concerns about the viability of the wing units of the shuttle but failed to communicate those concerns to NASA's top managers. Though the problem was discussed intensively via e-mail and telephone conversations, it was decided to not forward concerns.
