He/she can be a manager or supervisor of any kind as long as they can answer the required questions. Information to include: The person you interviewed, when, where, and why. Required Interview Questions 1. Define the change process or outcome you improved. 2. What data did you collect and how was it reported? 3. How did you know that change was an improvement? 4. What quality tools did you use? 5. Did you pilot test or segment the change processes? 6. What changes, if any, did you make once evaluation was complete? A summary of the information you gained from the interview. Personal reflections and conclusions that resulted from the experience and the knowledge you gained from it, and how you could use some of this knowledge in your own practice
