I have developed an outline for a "care team" structured approach, which is currently a significant focus in the Value-Based Care (VBC) and Quality domains of healthcare. I created this outline as part of an assignment for my Healthcare Administration Evidence-Based Practice course. So, it is not necessarily finalized.
