DATABASE & QUESTIONNAIRE DESIGN

Design a good questionnaire with 5 questions that would result in the type of data that we would need to build this churn model. Imagine that every single student at Neo Avant-Garde will take your questionnaire and that you will need to capture all the filled out forms and use the information to create a database.

Build something like a survey.

As part of your questionnaire preparation, answer the following questions:

Will your form be electronic or paper based? How many variables are in the training data set? What questions would you need to ask students in order to receive the data you need for this case study? How will students access your questionnaire? Will you ask open ended questions e.g., what after-school activities do you take? Will you ask closed questions e.g., do you take music as an after school activity?
