On Semiconductor is an electronics manufacturing company that produces different IC packages namely plastic and ceramics along with different lead types of 14 L, 18L, 24 L and the 40 L microprocessor. The last 2 quarters show an issue with the 24 Lead plastic package where its rejection level is increasing. Upon investigation, it was found out that 250 pcs have been rejected due to bent leads. The lead cutting machine has been set up wrongly by the technician while 150 pcs have been stamped with its product code but rejected due to substandard ink used for stamping. In addition, 200 pcs have been seen to have contamination while 125 pieces are rejected to issue in the normal temperature testing method. The packaging has been an issue with 2 batches composed of 300 pcs with wrong documents in it. It has been assumed that the staff has committed mistake in putting the correct documents.
