Briefly summarize the failure (no more than 1 page). In particular, answer the following questions: i. What is the source of your example? ii. What is the name of the software? iii. When did the failure happen? iv. What was the software failure? v. What was the software error that caused the failure? Proper notation must be used for quoting your sources.
