Case Study: Multi-story Construction Scaffolding Accident and Worker Fatality Date: 2 August2021 Place: 448B Baridhara West Avenue, Baridhara, Dhaka Five workers were injured after badly constructed scaffolding holding the workers while working on the building exterior fell from the ninth floor of a Baridhara commercial building. One worker died in the hospital later on in the day. The building was under Shymoli City Holdings, a building and construction firm located in 3345 Shymoli, Dhaka 1205. On the day of the accident, the deceased worker and five other co-workers were installing the steel reinforcement bars for a reinforced concrete beams. The on-site crane was non- operational due to engine trouble. This bar was located at the edge of the roof. The deceased, while handling one end of a bar, tripped and fell over the edge of the roof platform and landed on the ground, about 9.8m down. He was taken to the local hospital after site manager managed to arrange a transport after half an hour. Upon enquiry, site management found there were piled up materials, broken brickwork and open sacking on the construction site. Due to no stop building construction schedule, with two 12-hour shifts, there has been little time for cleanup and setting up of netting or ropes around the first rising construction floors. The safety netting is not ready for the whole building, and so rope harnesses are being used for now. The deceased worker had just finished his 12-hour shift and was assisting his co-workers, as the countrywide lockdown has reduced the number of available workmen for construction projects. Unskilled labor is now mainly available at lower salaries. The collapse of the scaffolding happened as other tried to grab their friend from the edge and over balanced the rope and metal scaffolding on they were all standing. The poor visibility at the time, given the rainy monsoon season, the slippery surface and lack of using hard helmets and rubber gloves led to their injuries. The workers staged a protest sit-in and work stoppage at the site for 24 hours, until this morning. Work will resume at site only after safety measures are evaluated and implemented. Q. Write an Incident Report in Memo format using company Letterhead, as the Site in Charge Officer who was present at the time of the incident, to Mr. Iftekhar Hossain, the Head of Operations of Shymoli City Holdings briefing him on the incident. Give recommendations to avoid further occurrences, improving site safety and advice on how to deal with worker unrest and tension at the building site.
