INSTRUCTIONS
Your health & safety consulting company has been hired by Millennium Transport Ltd to conduct a training Needs Assessment. Millennium Transport Ltd. manufactures and distributes tractor equipment used in heavy duty agricultural/forestry sectors and for general lawn care. Currently the company has a very limited training program however as they are attempting to achieve ISO 9001 certification, a robust training program is required. In addition, you have been informed that numerous incident reports have been filed over the past year and some have resulted in injures with lost time. Furthermore, some workers who do return to work are not able to work to 100% capacity of the jobs that they previously occupied.
You have been tasked to perform a Training Needs Assessment (TNA) based on the current situation and are looking to identify:
What is the itch/ problem statement?
What is the current state of the situation versus what is the desired situation that you want the company to be in?
What training is required to implement a robust training program?
What are the three main causes leading to these injuries?
What are the benefits associated with the recommended training programs
Once completed you and your team will submit a written report and present the findings outlining:
Information about the company and what its needs are (itches)
Identify the issues (i.e. what tasks are leading to the injuries)
Provide a description of the TNA tools to be utilized (i.e. questionnaire. Interviews, etc.)
Explain how the tools will be utilized and will lead to the development of training outcomes /learning objectives, based on the TNA
Include all job specific training programs required based on the TNA
Identify how the training will can help close performance gaps (i.e. will resolve the issues)
From a qualitative standpoint, provide a cost-benefit analysis associated with implementing a robust training program
Be creative and fabricate results. The goal is to understand how a Training Needs Assessment is done effectively and to practice completing one
MILLENNIUM TRANSPORT LTD. JOB DESCRIPTION
Janitorial your work.
Clean the assigned area in a distribution or fulfillment center
Duties will include cleaning and maintaining restrooms, break rooms, carpets, and hard surface floors by vacuuming, sweeping, and mopping floors
Operate floor scrubber (electric or propane) and other equipment as needed
Dusting racks, boxes, parts, ventilation, lights, and blinds including cleaning high and low spaces in distribution center or warehouse
Emptying wastebaskets, tying up bags, cleaning windows, and other normal cleaning and
maintenance duties
This role will require you to be constantly on the move for an 8-10-hour shift
Process trash removal and corrugated scraps with a compactor
Ability to lift / carry up to 50lbs
Ability to stand, bend, lift and / or reach for extended periods of time

INCIDENT REPORT SUMMARIES: JANUARY MAY
1. While dumping the mop pail at the end of shift, I slipped on the floor. I was fine when I left work, but in the morning my back was sore and I went to the walk-in clinic for a checkup. The doctor says I slipped a disk in my lower back.
2. While loading samples into my vehicle, I slipped on the wet hallway floor. I was not hurt but two containers of gear oil (15/40) broke open and spilled on the floor.
3. While vacuuming the office area, I went to move the electrical cord out of the way and was shocked. My hand had a small red mark and was sore; however I was able to continue working for the rest of the shift.
4. While cleaning the mens restroom, Kacey Green got some of the cleaner his hand causing it to burn. He then washed his hand with soap and water but it made the burn worse. The ambulance took Kacey to Emergency for treatment
