Instructions, Part Two Step 1: Create a Cause and Effect or Fishbone Diagram using at least 4 M's to help you find the root cause for: 1. Why the oil change at a car dealership was inadequate, OR 2. A customer was not happy with the haircut/style they just received Step 2: You must have at least 2 causes for each of the 4 M's selected. Refer to Week 6 for more information
