You are a student intern in an intensive case management agency in the Bronx, assigned to a client named Robin who needs help with several areas of her life. You have been meeting with Robin for two months now. Robin is a 27 year-old woman who has been dual-diagnosed with Substance Induced Psychotic Disorder, Cocaine (crack) Dependence and Borderline Personality Disorder. Robin receives SSD and food stamps monthly because of her mental illness. Robin refuses to take the psychotropic medications that she is prescribed.

Robin is presently in need of advocacy with her foster care agency and family court as she is litigating to maintain custody of her 2 year-old son Brian who was taken into custody immediately after Robin gave birth two years ago. Brian is currently living with his foster care mother; Robins other four children are also in the system and reside with their respective foster care placements. All of Robins children are still in foster care because of the continued substance abuse and neglect. All of Robins children have different fathers who she and the children have no contact with. Robin does not speak with any other members of her family. Robin has never met her father and doesnt know if hes alive or dead. Robin does not know the whereabouts of her two older brothers. Robins mother resides in the Bronx but Robin states, Shes fed up with me. We dont get along and used to fight all the time. Robins mother is the legal guardian for her first and second born children who she no longer allows Robin to see.

For the past eight months, Robin has been inconsistent with her visitations and complying with the courts mandates in order to regain custody of Brian. Robin is continuously in and out of either in Detox/Rehab Centers or on the Psychiatric Inpatient Unit of local hospitals. Robin admits herself willingly to these help centers but then gets agitated while there and signs herself out against the recommendations of the staff.

At times, Robin indicates to you that she doesnt want to lose Brian the way she lost the others because he is special and she wants to do it right this time. At other times, Robin tells you that shes hopeless and going to give up because Brian is probably better off with his foster care mother who wants to legally adopt him. Robin states, Im no good at being a mom anyway.

Recently Robin separated from her partner and roommate Trisha, after getting into yet another domestic dispute (You might call this relationship a co-dependent relationship). Robin tells you how she and Trisha got into another fight about drugs and that they were both high when this happened. Robin tells you that she is moving out of Trishas apartment and going to go live with a guy named Lester (age 53) who she met at her Day Treatment Program. Robin tells you he likes me and wants to be with me. Upon further questioning, Robin tells you that shes pregnant again and Lesters the father. Robin tells you that, Lester doesnt do drugs anymore and that hes good for me, except when hes not always telling me what to do. Robin is thinking about having an abortion but isnt sure if thats what she should do. Lester says he wants to have the baby, marry her and move to Brooklyn.

Robin says she takes life one day at a time; that is why she doesnt have long-term goals. Robin says she wants her own place unless that involves her doing a lot of running around. I dont have the patience for that. If it becomes too much trouble Ill just stay with Lester. When you talk to her about being inconsistent with her court mandates she says that she feels like youre becoming one of those social workers whos into appointments, where-were-you, and hassles like that. Just help me or leave me alone.
