Root Cause: The Difference Between the Symptom and the Sickness: After completing the Week 4 reading, using the required text reading, and posted articles, respond to the following:

1. In the context of an Organization Issue which requires an Organizational Development intervention, describe the difference between a symptom and the root cause.
2. Provide a real-world example of a situation you experienced where there was confusion regarding root cause. Provide details of the problem/issue, the initial symptoms which indicated the presence of an issue, the nature of the confusion during the assessment and your assessment of the true root cause.
3. Employing the 5 Why Technique, ask and answer the 5 Whys for the situation described above and walk through from symptoms to root cause.
