Draft the content of  feedback form or survey which can be sent out to clients to seek feedback on the range, type and quality of the services of ABC Bookkeeping. A rating scale is an effective way to gather information from customers.

You must produce your feedback survey in a separate document. Ensure it is to a standard that would be accepted in industry and is ready to be published.  You also need to include information on what you will use the data for and some instructions to the client on how they can return the feedback to your business.
