She is currently residing at a skilled nursing facility. She is here post hospitalization of encounter of orthopedic care and acquired deformities of the toe. She has Psych diagnosis of MDD, GAD. She is currently on Paroxetine 30mg at HS for MDD, Hydroxyzine 25mg every 12hours PRN and Vit D3 250mcg daily. Per her administration records, the patient has not been using the Hydroxyzine 25mg . stop date is 6/16/25. She said she knows she got the hydroxyzine order and she will only request it when in severe anxiety.
