Write an Initial PSYCH VISIT, chief complaint, , Psychiatric visit HPI, current Psych meds, exam, social history,, screening tools and results MSE, diagnosis and recommended Plan (medication and dosages), rationale for using the medications for an 77years old female who resides at a skilled facility currently seen today for initial psychiatric evaluation. He is here for post hospitalization of DISLOCATION OF INTERNAL RIGHT HIP PROSTHESIS, SUBSEQUENT ENCOUNTER. She HAS PSYCH history of GAD,MDD, panic attack and insomnia. She is currently on Citalopram 40mg (take 1.5tabs) for MDD daily. The assessment was done in the patient's room. She said she was doing ok. She said her leg aches and she just got back from rehospitalization last night where she stayed in the hospital for 3days. She said her mood is down because of her right hip fracture. She said she sleeps ok, has great interaction with her room mate. She verbaalised her anxiety is through the roof initially but she is gradually adapting. but her appetite is good
