Write an Initial PSYCH VISIT, chief complaint, , Psychiatric visit HPI, current Psych meds, exam, social history,, screening tools and results MSE, diagnosis and recommended Plan (medication and dosages), rationale for using the medications for an 93years years old male who resides at a skilled facility currently seen today for initial psychiatric evaluation. He is here for post hospitalization of metabolic encephalopathy. he has psych history of MDD and Insomnia. He is currently on Mirtazapine 7.5mg for MDD, Bupropion hcl 300mg for MDD and melatonin 1mg PRN for insomnia. Assessment was done in the patient's room. He said his mood was good just struggling with his healthy but he believes he will get some treatment here. He sleeps well, appetite is good and loves participating in social activities in the facility. He said when not here, he lives alone, a widower. He has 3 adult children, one is a doctor and two girls who care about him. Plan is to continue current meds
