Write an Initial PSYCH VISIT, chief complaint, , Psychiatric visit HPI, current Psych meds, exam, social history,, screening tools and results MSE, diagnosis and recommended Plan (medication and dosages), rationale for using the medications for an 79years old female who resides at a skilled facility currently seen today for initial psychiatric evaluation. The patient is here post hospitalization of intracapsular left femur fracture. She has psyh diagnosis of Bipolar, mild Cognitive impairment, alzheimers disease late onset, dementia disease, anxiety disorder and delusional disorder. She is currently on Amitryline hcl 10mg (take two tabs) at HS for depression as AEB by sleeplessness and tearfulness. She also takes Sertraline hcl 50mg at HS for depression . Her PHQ is 2. Family also requested psych referral. She said her problems started on 5/2/25 when she fell at the target store. She said it has been a month now since she fell and she has never been to her house . she said she has been in the facility for way too long and she is very lonely here. She had episodes of anxiousness, crying and laughter during this assessment. She said things got worse she they gave her a bried to were and she had to pie on herlsef which is very akward for her.
